Patient Info

Welcome New and Returning Patients

We are dedicated to offering our patients the best care and to provide you with all of the important information you need to make healthy decisions.

Appointments

The first step towards dental and vision healthcare is to schedule an appointment. We provide our patients with the convenience of both options at the time of your office visit.

What you can expect from your first visit?

Your first vision appointment includes a comprehensive exam along with a refraction test that helps check the health of your eyes and determine your prescription.

Your first dental appointment generally includes an initial exam, x-rays and a cleaning. Your dentist will discuss your diagnosis and treatment options. If you are experiencing tooth pain, our dentists will evaluate the site area and determine the best course of action to alleviate the pain.

Please assist us by providing the following information at the first visit:

  • If your previous dentist has taken x-rays, please request that they forward them to our office.

  • List of current medications

  • Photo ID

  • If you have dental or vision insurance, please bring your insurance card so we are able to process your claims.  All insurance information is provided to our office at the time you schedule your appointment so staff can verify your benefits prior to your scheduled appointment.

Insurance Information

If you wish for us to bill your insurance company and have your benefits payable directly to our office, you must provide us your dental and vision/medical insurance information (card) to verify your eligibility and benefit provisions before your first scheduled appointment. Otherwise, you will be responsible to make full payment for services rendered and we will submit the insurance claim on your behalf to have the benefit payments sent directly to you.

Your vision benefits can be used for more than an annual eye exam and prescription eyeglasses. Vision insurance and FSA dollars also cover sunglasses made with your prescription, and contact lenses - everything you need to see and look your best, all year long.

We accept most major insurances, dental & vision medical plans. Call our office at (860) 956-1396 to check and see if we are a provider for your plan!

Payment Options

For your convenience, we accept Visa, Mastercard and Discover. We also accept the CareCredit card for dental. Payment is due at the time of service unless prior arrangements have been made with our office. We do not accept personal checks. Please call us at (860) 956-1396 if you have any questions regarding your account.
